The Utah State Court System has forms for filing for a protective order. The Utah Online Court Assistance Program will also let you fill out the forms online by using the step-by-step program.

- Cover Sheet for Civil Actions

A domestic violence organization in your area may also be able to provide you with the necessary forms.
