Tennessee Court Forms

January 8, 2019

The Tennessee Courts website has some of the forms you might need, including petition for an order of protection, petition for a temporary order of protection, and a motion to modify or extend an order of protection.

The Tennessee State Courts website also links to parenting plan forms as well as a guide to developing a parenting plan. You can also find forms for divorce and other civil cases.

To find the courthouse in your area, go to our TN Courthouse Locations page. Someone in a domestic violence agency in your area may be able to help you get the forms and also help you through the process. To find an agency in your area, go to TN Advocates and Shelters.
