Court forms are available on the South Carolina Judicial Department website. You can find the petition for a domestic violence order of protection and the “notice and motion” to extend a domestic violence order of protection. To find forms related to custody, visitation, and other family court issues, scroll to “family court” in the first drop-down menu.

You can also find the form called “complaint and motion” for a restraining order based on harassment and stalking as well as a motion to extend a restraining order based on harassment and stalking.

You can also find the forms you need at the courthouse. To find the courthouse in your area, go to our SC Courthouse Locations page.
