Nevada Court Forms

Updated:
October 25, 2023

The Nevada Courts website has forms that you can print out for orders for protection against domestic violence, sexual assault, high-risk behavior, stalking or harassment, and harassment in the workplace, divided by Clark County, Washoe County, and all other counties

Alternatively, you can ask the clerk to provide you with the forms when you arrive at the courthouse. See our NV Courthouse Locations page for courthouse contact information.

To learn more about the process, please visit Steps for obtaining an order of protection.

Someone at a domestic violence organization may also be able to help you get the forms and can help you through the process. To find a shelter or an advocate at a local program, please visit the NV Advocates and Shelters page.
