The Utah State Court System has forms for filing for a protective order. The clerk of court can help you determine which forms you need.
You may need some of the following forms and instruction sheets:
- Verified Petition for Protective Order
- Verified Petition for Protective Order on Behalf of Children
- Cohabitant Abuse and Domestic Violence Instruction Sheet
- Affidavit for Filing Foreign Protective Order
- Cover Sheet for Civil Actions

A domestic violence organization in your area may also be able to provide you with the necessary forms.
