Preparing for Court

Certain forms that you may need to file a court action in New York are available online from the State of New York Unified Court System.
The clerk's office at the courthouse will also have the forms you will need and where you will file. (NY Courthouse Locations)
You may also be able to obtain them by calling a local domestic violence agency or legal aid office. To find a shelter or an advocate at a local program, please visit the NY State and Local Programs page under the Where to Find Help tab at the top of this page.
If you are in New York, Kings, Bronx, Queens, Richmond (Staten Island), Albany, Rensselaer, Westchester, Suffolk, Orange, or Schuyler counties and are filing for an Order of Protection , you may be able to use one of the interactive computer programs designed to help walk you through the process of filling out the forms at a shelter there.
