Some of the forms you will need are available on the KY Courts website. To access domestic violence forms, click on "Catagorically" under "Electronic Forms". Then click on "Domestic Violence".

You will also find forms at the courthouse in your area. Go to the Kentucky Courthouse Locations page to find the courthouse in your area. Ask the civil court clerk to help you get the forms you need.
Your local domestic violence organization may also be able to provide you with the necessary forms. Go to KY State and Local Programs to find help in your area.
