You can find forms from the Indiana Courts website.
To download a form, click on the Word or PDF link to the right of the form you need. If you click on the "Word" link, the form will save as a Microsoft Word file. If you click on the "PDF" link, the form will save as a .pdf file, which you can open using Adobe Reader. Click here to download Adobe Reader for free.
You can also check with the Clerk of Court at the courthouse in your area to find the forms you need.
A domestic violence organization in your area may also be able to provide you with the necessary forms. Go to IN State and Local Programs to find help in your area.
