Hawaii issues protective and restraining orders through both the District Court and Family Court. See the How to Get an Order of Protection page to determine which court you should go to.

Forms for Family Court protective orders are not yet available online, but check the links below in case they have been posted. For District Court restraining orders, you will find these forms online for each judicial district:
